Give IV fluid immediately Ringer's Lactate (IV) 200 ml/kg as follows
age below 1 year |
100 ml/kg over 6 hours 30 ml/kg in the first hour 70 ml/kg over the next five hour |
age above 1 year |
100 ml/kg over 3 hours 30 ml/kg within half an hour 70 ml/kg over the next two and half hours. |
Monitor frequently; give ORS in addition to IV fluids as soon as able to drink.
Reassess after 4 hours; if improved, treat as moderate dehydration, if still severe, continue IV fluids.

Doxycycline (O) 300 mg as a single dose or 5 mg/kg |

Tetracycline (O) 500 mg four times a day for 3 days (or 50 mg/kg/24 hours in 4 divided doses). |
Alternative in children <8 years |
Co-trimoxazole (O) 48 mg/kg/24 hrs in 2 divided doses for 3 days. |

Trimethoprim (O) 200 mg twice daily for 3 days |
