4.6.2 Frequent Attacks

Drug of choice	*Isosorbide dinitrate (O)* 30-120 mg/day in 2 divided doses. If no response,
	Add
Second line	*Propranolol (O)* 40-80 mg three times a day
	OR
	*Atenolol (O)* 50-100 mg once daily.

Atenolol is preferred for diabetics and asthmatics. If there is no response to the combination of nitrates and beta blockers try:

Nifedipine (O) 10-20 mg three times a day (it may replace or be cautiously combined with beta-blockers).

NOTE If pain continues inspite of above treatment: refer for farther investigation and treatment.
